VHB Healthy Mobility Tool - Risk Assessment Dashboard

VHB's Healthy Mobility Tool was designed to provide an opportunity for identifying areas across the United States which may be at risk for the occurance of six chronic diseases. Following a statistical process, each of the six diseases were evaluated to determine the level of risk within census tracts using a variety of available demographic data and information. As part of this risk assessment, each of the six diseases were considered and an overall Risk Score was determined. Please contact members of the VHB Healthy Mobility Team to learn more.
